1) Legal Name, Home Address,and Ownership Percentage for each Member (owner) of the LLC:

2) Proposed Company Name of the LLC, and the type of business:

3) Mailing Address and Phone Number for the LLC (no PO Boxes), and who will be listed to receive your mail (your Registered Agent - you may serve as your own Registered Agent, but you do not have to):

4) SSN. From any one Member. I’ll use this to get your new EIN Number (your Federal Tax ID Number) from the IRS.

5) Officer Info - do you want any formal titles? President/VP/Secretary, etc. This is optional.

6) Payment info.  We accept Check, Cash, e-pay app, Credit Card. No taxes or ongoing fees, it’s a simple one-time payment.

Our LLC Formation Package is $1,275 per Company formed, minus your $100 Discount for finding us online (any additional LLCs or Estate Planning in the future will be discounted $200 Off).  If you are comfortable doing a nice Google Review for us, we always appreciate it (link below)!

This includes everything. I will pay your state filing fees and retrieve your EIN Number from the IRS.  This price includes all materials, filing fees, shipping fees, legal fees, custom corporate docs, Company Agreement (operating agreement), Meeting Minutes, Ownership Certificates, Corporate Seal, etc. Everything you need to go to a bank to open your new business bank account.
